Pre-operative Cardiovascular evaluation
For Physicians/Physician’s Office/Hospital:
Dr. Hudson performs full pre-operative cardiovascular evaluations for non-cardiac surgeries for both new and existing patients. We offer in-office nuclear cardiology and ultrasound services which allow us to evaluate patients in a timely manner.If you are a physician, or from a physician’s office or hospital and need pre-operative clearance on a patient, please call us at 770-887-0472.
Each patient will be evaluated for the risk of a cardiovascular complication by performing an office visit and/or diagnostic testing.
In order to properly evaluate the patient, we will need the following information to complete the cardiac clearance letter.

Please return the completed form to us by fax at 770-887-1140.
Once the cardiac clearance has been completed by our office, we will then return the cardiac clearance and any supporting clinical to the requesting surgeon as indicated on the form.
For Patients:
If you have an upcoming non-cardiac surgery scheduled and have been advised that you will need a cardiac clearance, please call our office as soon as possible at 770-887-0472.Early notification allows us time to schedule an appointment and any diagnostic testing in the most timely manner.
At time of your call to us, please be prepared to provide our office with the name, phone and fax number of the surgeon requesting the cardiac clearance.
